Identification and Appointments
Patients are required to present two pieces of valid identification at their initial appointment.
Patients who are unable to attend a scheduled appointment must provide at least 48 hours’ notice to cancel or reschedule.
Late cancellations or missed appointments may result in a $45 fee.
Repeated late arrivals, late cancellations, or missed appointments may affect the availability of future appointments.
Consultation and Treatment Recommendations
Following the initial consultation, Dr. Wang will provide professional recommendations based on the patient’s health concerns, medical history, available laboratory results, diagnostic information, and treatment goals.
Recommended treatment options, expected treatment duration, and all applicable fees will be discussed before treatment begins.
Patients will have an opportunity to review the recommendations, ask questions, and decide whether they wish to proceed.
Treatment plans are individualized and may vary according to each patient’s needs, clinical circumstances, and response to care.
Payment and Insurance
Payment may be made by:

A properly issued receipt containing the clinic’s registration information will be provided after each payment.
Longevity Fertility Centre does not offer direct billing and does not communicate directly with insurance providers regarding claims.
Patients are responsible for submitting their own receipts and managing all insurance claims directly with their insurance provider.
Insurance coverage varies between plans. Patients are encouraged to confirm their eligibility and coverage before receiving services.
Refund Policy
Traditional Chinese Medicine herbal formulations are customized specifically for each patient.
Once a customized formulation has been prepared or ordered, it cannot be returned, exchanged, transferred to another person, or refunded.
Consultation fees and treatment fees are non-refundable once the applicable professional service has been provided.
Health products and other customized items are non-refundable once they have been prepared, ordered, or supplied.

Longevity Fertility Centre is committed to providing professional, ethical, and high-quality Traditional Chinese Medicine care.
Traditional Chinese Medicine services are intended to complement, not replace, appropriate medical assessment, diagnosis, and treatment provided by a family physician, medical specialist, fertility clinic, or other regulated healthcare professional.
Patients should continue to follow the recommendations of their medical providers and seek appropriate medical attention whenever necessary.
Individual responses to treatment vary. Longevity Fertility Centre cannot guarantee pregnancy, live birth, or any other specific treatment outcome.
Respectful Clinical Environment
Longevity Fertility Centre is committed to maintaining a professional, safe, and respectful environment for patients, practitioners, and staff.
Patients are expected to communicate respectfully and refrain from:
- Threatening or abusive behaviour
- Harassment or intimidation
- Discriminatory or hostile conduct
- Conduct that creates a safety concern
- Repeatedly disruptive or inappropriate communication
Repeated late arrivals, late cancellations, missed appointments, unpaid balances, inappropriate conduct, or a serious breakdown in the therapeutic relationship may affect the clinic’s ability to continue providing care.
Where appropriate, concerns will be discussed with the patient before future appointments are declined or care is discontinued.
Immediate discontinuation of care may be necessary when conduct involves serious threats, harassment, abuse, or an immediate safety concern.
