Longevity Fertility Centre operates based on appointment only.
Patient will need to contact us and book an appointment in advance.
Patient will be asked to proceed with advance payment for the consultation fee before booking.
The cost for initial consultation for one hour appointment per person is $125 (before GST).
After receiving the consultation fee, the office will send the patient questionnaire form to fill out and confirm the appointment date.
