Patients are required to present two (2) pieces of valid identification on the date of their appointment. If an appointment cannot be kept, patients must notify the Centre at least 48 hours in advance to cancel or reschedule. Late cancellations or failure to attend a scheduled appointment (no-show) may result in a $45 fee.

Dr. Wang will provide appropriate professional advice following the initial consultation. Treatment options and applicable fees will be discussed directly with the patient prior to proceeding. Please note that Longevity Fertility Centre does not offer direct billing.

Payment may be made by cash, Interac, Visa, Debit, E-Transfer, or MasterCard. A properly issued receipt with a government-registered number will be provided after each payment.

All herbal medications are customized for individual patients and are therefore non-refundable. This no-refund policy applies to all health products and services provided by the Centre. Longevity Fertility Centre does not deal directly with insurance providers; patients are responsible for submitting and managing their own insurance claims.

Longevity Fertility Centre maintains a zero-tolerance policy toward verbal abuse, hostile behavior, or defamation directed at staff members. Repeated late arrivals, late cancellations, unpaid fees, or any form of misconduct may result in refusal of service or termination of care at the discretion of management.
